This position involves deliveringcomprehensive medical care, managing chronic and acute conditions, andoptimizing the well-being of patients as they transition from acute care topost-acute care. The PA will collaborate with interdisciplinary teams to ensurepatients receive high-quality, personalized care and support their recovery.
