HomeCare Elite. A DAY IN THE LIFE OF A SPEECH LANGUAGE PATHOLOGIST
Under the direction of the Clinical Manager, the Speech-Language
Pathologist (SLP) provides skilled speech therapy services to
patients in their homes as part of an interdisciplinary care team.
The SLP evaluates patients, develops individualized treatment
plans, and provides therapeutic interventions to address
communication disorders, swallowing difficulties, and
cognitive-linguistic impairments resulting from injury, illness, or
disability. Responsibilities include but are not limited to: KEY
RESPONSIBILITIES: •Performs a comprehensive speech and language
physical, psychosocial, and safety assessment as evidenced by
documentation in the clinical record. •Perform initial and ongoing
assessments of patient's communication and swallowing status.
•Develop and implement individualized treatment plans based on
physician orders and patient needs. •Provide direct speech,
language, voice, fluency, and swallowing therapy services in the
patient's home. •Instruct and educate patients, caregivers, and
family members regarding goals and home exercise programs.
•Collaborate with interdisciplinary team members (nursing, PT, OT,
social work) to ensure holistic and coordinated care. •Monitor and
document progress toward established goals and adjust plans as
necessary. •Communicate regularly with physicians and other health
care professionals regarding patient status and progress.
QUALIFICATIONS: •Master's Degree in Speech-Language Pathology from
an accredited program. •Current licensure as a Speech-Language
Pathologist in the state of practice. •Minimum of two years of SLP
experience preferred; home health experience a plus. •Certificate
of Clinical Competence (CCC) from the American
Speech-Language-Hearing Association (ASHA) required. •Valid
driver's license, auto insurance, and reliable transportation.
